"Everyone" is an understatement. Loads of people end up in hospital because of edgeplay and bloodplay and asphyxiation stuff and they don't have a safeword or a safeword equivalent because they don't think of it or they trust their play partner enough to think they don't need one because their partner will know when to stop.

A john wanted to do heavy S&M play with me without a safeword once. Madame's security landed him in the hospital. They didn't give him a safeword.
